Collaborative Planning and Implementation (CPI) initiative groups are composed of stakeholders in a county or region that work together to identify, discuss, and resolve topical implementation issues and identify funding initiatives that may be used to address implementation gaps and improve outcomes. Chapman Consulting facilitates the San Francisco and South Bay Collaboratives.
Chapman Consulting is an approved vendor on the Technical Assistance (TA) Marketplace, a virtual marketplace where providers, community-based organizations, counties, and others can obtain technical assistance resources to establish the infrastructure required to implement Enhanced Care Management (ECM) and Community Supports.
